The PVH Patient Registration Department is located off the upper parking lot outside PVH, off Transalpine Road. Each patient, unless brought in by ambulance, must first visit the patient registration to fill out paperwork and answer some preliminary questions.

What to bring
It is important to bring any insurance cards that you may have. This will help speed the registration process. For security reasons, please bring with you a photo-identification, such as a driver’s license or Maine State ID card. Also, if you have an order from your doctor for a hospital service, please bring that with you, as well.
We can help
Some of the services we can provide at patient registration include help with a living will and applications for MaineCare. For your convenience, you can also make a payment toward your hospital bill at patient registration.
