Case Management
The Case Management Department acts as a resource to the patients, the medical providers, the hospital, and the community. Its role is to facilitate effective follow-up and wellness after discharge from the facility. The case Management team works with all hospital departments in the process of assessment, planning, accessing, coordinating, and implementing care for the patient.
Included in Case Management are the Social Services, Utilization Review, and collaboration efforts. PVH is growing and an anticipating many new projects and goals to continue and ensure patient care at the hospital.
